How to Refer a Patient

Contact Our Office

Call (947) 274-8300 to initiate a referral. For urgent cases (Mohs defects, trauma, cancer), please indicate the urgency and we will prioritize scheduling.

Send Relevant Records

Please fax or securely send: referral letter, relevant medical records, imaging (CT scans, X-rays, MRI), pathology reports (for cancer cases), and photos of the condition or defect. Fax: (947) 274-8301.

Patient Consultation

Dr. Chaiyasate will see the patient for a comprehensive consultation, perform an examination, review imaging, and discuss treatment options. Urgent referrals are seen within 24–48 hours; routine referrals within 1–2 weeks.

Communication Back to You

You will receive a detailed consultation note outlining the diagnosis, recommended treatment plan, and surgical timeline. After surgery, you will receive the operative report and follow-up plan.

For Mohs Surgeons & Dermatologists

Dr. Chaiyasate has extensive experience in facial reconstruction after Mohs surgery and welcomes referrals from Mohs surgeons and dermatologists. He offers:

Same-day reconstruction when possible — coordinated with your Mohs schedule
24–48 hour consultation for all Mohs referrals
Full spectrum of reconstruction: primary closure, local flaps, regional flaps, skin grafts, staged procedures, and microvascular free tissue transfer for large defects
Expertise in complex nasal, eyelid, lip, and ear reconstruction
Detailed operative reports and photos sent to your office
Patients returned to your care for ongoing skin cancer surveillance
